To switch medical plans or add/drop dependents complete the Medical Enrollment Form below.
To switch dental plans or add/drop dependents complete the appropriate Dental Form below.
No forms are required for the VSP PPO Vision Plan. You and your dependents are automatically enrolled.
FLEXIBLE SPENDING ACCOUNTS (FSA)
Complete the FSA Enrollment Form to participate in 2017. Current participants must re-enroll to continue for 2017.
The annual amount you elect will be converted to twelve (12) payroll deductions a year. If you are paid less than 12 times, you will have an arrears balance that will catch up on the next available paycheck.
