The following two forms are required to be completed for all student injuries and/or accidents that occur on-campus and/or off-campus during a college sponsored event or activity:

1) Student Accident/Illness Report Form

2) HIPAA Individual Authorization Form (Anthem) & Student & Athlete Insurance Network - Accident Claim Verification Form

To report an incident, please contact Francys Ropohl, Health and Safety Specialist. Also report all incidents to your instructor or supervisor.
