Student Insurance


The following two forms are required to be completed for all student injuries and/or accidents that occur on-campus and/or off-campus during a college sponsored event or activity:

1) Student Accident/Injury Form

2) HIPAA Individual Authorization Form (Anthem) & Student & Athlete Insurance Network - Accident Claim Verification Form

3) To Find an In-Network Provider (instructions):

     a) Go to https://www.anthem.com/ca/find-care/

     b) Select Basic search as a guest and under the following, select one, then click Continue:

         > Select the type of plan or network - select Medical Plan or Network et al.
         > Select the state where the plan or network is offered - select California
         > Select how you get health insurance - select Medical (Student Health)
         > Select a plan or network - select Blue Cross PPO (Prudent Buyer) - Student Health

     c) Click on Update Location button, then enter Zip Code, City or County

     d) Under Search by Care Provider - select an option*:

          > Primary Care
          > Behavioral Health
          > Lab (Blood Work)
          > Imaging (MRI or X-ray)
          > Hospital
          > Urgent Care

          *Note:  Options vary by location

Report an Incident:
